You will notice that the Family History section of your templates has increased from 1 field to 3.  This change was required by Meaningful Use and incorporates the use of SNOMED codes in each field for tracking purposes.

The new fields look like this:

These fields will not autofill from visits prior to 2015 and they must be addressed for each visit.  If there are no family history items to note, this must be documented.
