Requirements:

1. Certified EHR - AdvancedMD EHR is ONC 2014 Certified

2. Reporting period - 90 consecutive days or a full year

Base Score Requirements (50% of total)

1. Security Risk Analysis

   1. Action: Complete a Risk Analysis and maintain in your office a signed and dated copy along with a copy of AdvancedMD’s Help Files on System Security and HIPAA Privacy and Security Rules by HITECH.

   2. Minimum - Yes

2. e-Prescribing

   1. Action: Using AdvancedMD’s ePrescribing, the drug formulary is enabled and the eligible clinician signs and eSends permissible prescriptions.

   2. Minimum - 1 patient

3. Provide Patient Access

   1. Action: Using AdvancedMD’s Patient Portal, your patients have access to their health information in real-time on demand and have the ability to view, download and transmit their information to a third party.

   2. Up to 20% Performance Score

4. Health Information Exchange

   1. Action: Use AdvancedMD’s Outbound CIE Tool to generate a Referral/Transition of Care record and electronically transmit it to another provider of care via Direct Messaging.

Performance Score Requirements (in addition to base score requirements, up to 90%)

1. View, Download or Transmit

   1. Action: Within AdvancedMD PM or EHR, the eligible clinician can send secure messages to a patient and can receive messages from the patient when assigned to one or more Message Distribution List(s) for Patient Portal messaging.

   2. Score: Up to 10%

2. Patient Specific Education

   1. Action: AdvanceMD’s patient education chart tool enable the provider to share patient education with the patient during the visit or via the Patient Portal.

   2. Score: Up to 10%

3. Medication Reconciliation

   1. Action: In AdvancedMD EHR, eligible clinicians are able to input, update, and reconcile medications using the Import Medication History feature, the Medications tab, or the Inbound CIE Tool and check the List Reviewed box.

   2. Up to 10% Performance Score

4. Immunization Registry Reporting (up to 15%)

   1. Action: In AdvancedMD PM, utilize the Health Agencies Module to transmit Immunization data. To facilitate electronic transmission to registries 1) Contact your state registries to declare your intent and to determine their readiness, 2) Contact AdvancedMD to setup registry connection.

   2. Score: Up to 10%

Bonus Scoring Requirements (in addition to base score requirements)

1. Syndromic Surveillance Reporting

   1. Requirement: The MIPS eligible clinician is in active engagement with a public health agency to submit syndromic surveillance data. Earn a 5 % bonus in the advancing care information performance category score for submitting to one or more public health or clinical data registries.

   2. Action: In AdvancedMD PM, utilize the Health Agencies Module to transmit Syndromic Surveillance data. To facilitate electronic transmission to registries 1) Contact your state registries to declare your intent and to determine their readiness, 2) Contact AdvancedMD to setup registry connection.

2. Specialized Registry Reporting

   1. Requirement: The MIPS eligible clinician is in active engagement to submit data to specialized registry. Earn a 5 % bonus in the advancing care information performance category score for submitting to one or more public health or clinical data registries.

   2. Action: Each Specialized Registry requires different data to be collected and custom programming to connect to it. For this bonus measure ensure you are collecting the appropriate data and use the registry’s portal to report your findings.
